After a training: whether people feel sure they can use it at work, not just whether they enjoyed it.

The conviction read: a room can agree it was a great day and still land slow and hedged on 'without my notes'. A show of hands at the close hides that gap, and the gap is where the follow-up help should go.

Questions about this template
Should I send this at the close or later?
At the close, while the day is fresh, then again on a fresh copy a few weeks later. The second round shows whether confidence on 'without my notes' held up once people were back at work.
What if 'back me in using it' lands low?
The training may be fine and the workplace may be the obstacle. Share that result with the managers of the people trained, and agree with them what support looks like before anyone is expected to use it.
