Cancellation reasons check
Sorry to see you go. A few statements about why you are leaving, so we can learn from it.
The conviction read: a menu reason gives one cause, and many people leave for several. The spread across 'more than it was worth to me' and 'suits me better' shows how much each weighed, which a single dropdown flattens.

Questions about this template
Why statements instead of a list of reasons?
People often leave for more than one reason. Answering each statement on its own lets someone lean a little toward cost and a lot toward something better elsewhere, so you see the mix, not a single click.
When should I ask?
Close to the moment someone leaves, while the reasons are clear in their mind. Keep it easy to ignore: someone on the way out owes you nothing, and a pushy ask can undo the goodwill you still have.
