Exit check-in
As you move on: how your time here looked from where you sat, to help the team that stays.

The conviction read: people on their way out tend to stay polite to protect a reference. A slow, hedged agree on 'My manager helped me do my best work' says more than kind words in an exit chat, more so when it repeats.

Questions about this template
Who sees the answers?
Whoever owns the copy sees its results. Say plainly in the intro who that is and who the results will be shared with, so each person can decide what to tell you.
Should the leaving person's manager send it?
Better someone outside the reporting line, such as a people team, since one item is about the manager. Keep several leavers on the same copy before reading the manager item as a trend.
