How often should the group answer it?
Every few episodes, or at the end of each season. Use a fresh copy each time, made with Duplicate from your survey list, so you can compare rounds and see interest slipping before people stop showing up.
What if 'I can follow the story without a recap' lands low?
Some viewers are likely lost. Open the next session with a short recap or slow down, and check whether 'getting through episodes at the right pace' moves along with it next round.
